A characteristic paradox: the patient gains weight due to fluid retention but loses muscle mass. This cardiac cachexia reflects the heart’s inability to properly nourish the tissues. When there is a persistent long-term weight loss despite visible edema, it is a reliable indicator of advanced phase.
Heart rhythm disturbances and irregular breathing
Heart rhythm disturbances become more frequent and less responsive to treatments. Cheyne-Stokes respiration is also observed, characterized by cycles of apnea followed by deep breaths, especially at night. This respiratory pattern often worries relatives who discover it at the patient’s bedside.

The advantage of these tools is that they do not depend on sophisticated cardiac examinations. A general practitioner or a coordinating nurse can apply them.

Feedback varies on the sensitivity of these grids depending on the patient’s profile, but their growing adoption in recent years reflects a shift in approach: we no longer just wait for acute failure to discuss end-of-life issues.
Repeated hospitalizations and advanced heart failure: when the pattern changes
A criterion rarely discussed in public content concerns the rhythm of hospitalizations. Two or more hospitalizations in six months for cardiac decompensation, despite adjusted medication treatment, constitutes a strong signal of progression towards the terminal phase.
It is not only the frequency that raises alarms, but the quality of recovery. When the patient leaves the hospital with a functional level lower than what they had before the previous admission, the curve descends without a stabilization plateau. Care teams then refer to it as refractory heart failure.
